What Is HyMAP?
HyMAP is ChroniTrack Health's flagship community program. It exists because the biggest health crisis in Nigeria isn't treatment; it is detection. Millions of people, especially in the informal economy, have no idea they have hypertension, diabetes, or other chronic conditions. By the time they find out, the damage is already done.
HyMAP goes to where these populations live: markets, settlements, informal hubs and brings screening to them. Free. No appointment. No hospital visit required. Just a blood pressure cuff, a pharmacist who speaks their language and a pathway to real follow-up care.
How HyMAP Works
Step 1: Screen
Pop-up screening clinics set up in markets and informal settlements. Blood pressure, blood glucose, and basic vitals checked on the spot. Free for all participants.
Step 2: Educate
Pharmacists and health educators conduct counseling in English and Nigerian Pidgin. Hydration, diet, lifestyle. Practical advice people can actually use.
Step 3: Map
All screening data is collected and mapped geographically. This creates a hypertension heat map of the community; showing exactly where chronic disease burden is highest.
Step 4: Follow Up
Patients who screen positive are connected to BPTracka for ongoing monitoring.
The screening doesn't end at the event, rather it begins a lifelong care pathway.

Why Hypertension?
Hypertension is the single biggest killer in Nigeria and most people don't know they have it.
54% of Nigerian adults have hypertension, but most are undiagnosed
The informal economy is the hardest-hit population; no regular check-ups, no insurance, no time
Unmanaged hypertension leads to stroke, heart attack, kidney failure and early death
Early detection + consistent management = dramatically better outcomes
HyMAP targets this gap. Not with awareness campaigns. With actual screening, actual data, and actual follow-up care.
